How to Fill Out the Subscription Form

Step 1: Choice of Options

Choose either the Individual Option or the Group Option by checking the appropriate box.

Individual Option

1. Enter your personal information and payment method in the Individual Subscriber or Group Representative section.
2. Specify the date on which you would like your subscription to start, and sign the form.

Group Option

1. Enter the Group Representative’s personal information, as well as the payment method selected, if applicable, in the Individual Subscriber or Group Representative section.
2. Enter each group member’s personal information, as well as the payment method, if applicable, in the Group Members section.
3. Specify the date on which you would like your subscription to start, and sign the form.

Step 2: Choice of Subscription

In this section, you must first decide whether you want to subscribe to the Trio lottos package or if you prefer a personalized subscription.

How to Subscribe to Trio lottos - Table A

(Individual or Group Option)

1. Decide on the desired length of your subscription (26 or 52 weeks).
2. Enter the quantity of Trio lottos packages desired (maximum of 10).
3. In the SLIPS section, enter your selections or choose Quick Pick.
4. Calculate and enter the total amount of the subscription.
5. Carry this amount to Step 3.

To Customize your Subscription - Table B

(Individual or Group Option)

1. Choose between a 26 weeks or a 52 weeks subscription (regardless of the duration selected, the duration will apply to all products chosen).
2. Determine the lotteries to which you wish to subscribe and enter the number of selections (maximum of 10) that you wish to have for each draw.
3. In the SLIPS section, enter your selections or choose Quick Pick.
4. Calculate and enter in Column A the partial cost of participating in each of the lotteries.
5. Determine the lotteries for which you wish to add the Extra numbers (maximum of 10). Indicate how many you wish to have for each draw.
6. Calculate and enter in Column B the partial cost for playing Extra in each of the lotteries.
7. Calculate and enter the total cost of participations in the lottery and the Extra for each of the lotteries (column A + column B).
8. Calculate and enter the total cost of your custom choice and carry this amount to Step 3.

Step 3: Summary of Amounts Payable  

(Individual or Group Option)

In this section, simply enter the totals of the appropriate sections and calculate the final total for the subscription.

1. Enter the total for the subscription to the Trio lottos package, if applicable.
2. Enter the total for the 26 or 52 weeks personalized subscription, if applicable.
3. Calculate and enter the total amount of the subscription.

Step 4: Methods of Payment

Individual Option

In this section, you must choose the method of payment: deferred payment plan or single payment.

1. Choose the desired method of payment in accordance with the subscription you have chosen.
2. Calculate and enter the amount or amounts in accordance with the chosen method of payment.

Group Option

For the Group Option, only the single payment method is accepted.

1. Carry the total amount of the subscription entered at Step 3.

Before sending the duly completed form to Loto-Québec, remember to:

• Select the non-profit organization to whom the commission will be paid from the list of accredited organizations [PDF Format, in French only, 122 Kb – Help].
• Include your payment or enter your credit card number, if applicable.
• Sign the form.
